Admission

People are generally referred to Clare Holland House by another health service – this may be a GP or specialist doctor, a hospital or a health service. Other people will be referred to Clare Holland House when it becomes apparent that home-based palliative care or community health services are no longer the best way of treating their life limiting illness.

This directory has been formulated to assist the community and health professionals access information about palliative care services, primary care services providing care at the end of life and a range of other services providing support to people with a terminal illness.
